Key Responsibilities
- Assess the needs of IDD participants and create individualized care plans in collaboration with clinicians, families, and community partners.
- Coordinate and monitor the delivery of health, social, and support services, ensuring compliance with state and federal regulations.
- Serve as the primary point of contact for participants, families, and service providers, addressing concerns and facilitating communication.

Qualifications
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related field (Master’s preferred).
- Minimum 2 years of experience coordinating care for individuals with intellectual or developmental disabilities.
- Strong knowledge of Medicaid, HCBS, and Florida IDD regulations.
- Excellent communication, organizational, and problem‑solving skills.
- Proficiency with case management software and Microsoft Office Suite.
- Valid Florida driver’s license and ability to travel locally as needed.
