Key Responsibilities
- Conduct comprehensive assessments of clients’ behavioral health needs and re‑entry challenges.
- Develop individualized care plans that coordinate medical, psychiatric, and social services.

- Monitor client progress, adjust interventions, and document outcomes in compliance with HIPAA and company standards.

Qualifications & Requirements

- Current licensure or certification applicable to Kentucky (e.g., LCSW, RN, LMFT) is a plus.
- Strong knowledge of community resources in Graves County and surrounding areas.
