About This Position
Broad River Rehabilitation is hiring a full‑time Speech‑Language Pathologist (SLP) to work at its Skilled Nursing Facility in Sparta, Monroe County, WI. The role offers a fixed annual salary of $151,819.94 and split duties with a second location in Tomah, WI.
Key Responsibilities
- Assess and diagnose speech, language, swallowing, and cognitive‑communication disorders for residents under physician orders.
- Develop and implement individualized therapy plans using evidence‑based techniques and assistive devices.
- Document evaluations, progress notes, and outcomes in the electronic health record.
- Collaborate with nursing staff, physicians, occupational therapists, and families to ensure coordinated care.
- Provide education and training to staff on communication strategies and safe swallowing practices.
- Maintain compliance with state licensing, Medicare/Medicaid regulations, and facility policies.
Qualifications & Requirements
- Master’s degree in Speech‑Language Pathology from an accredited program.
- Current state license as an SLP and required SLP‑CCC certification.
- Eligibility for certification by the American Speech‑Language‑Hearing Association (ASHA) preferred.
- Minimum 1‑2 years of clinical experience in a skilled nursing or acute care setting.
- Valid driver’s license and reliable transportation for travel between Sparta and Tomah facilities.
- Excellent communication, interpersonal, and documentation skills.
