Trinity Rehab Services is seeking a Licensed Physical Therapist to provide skilled care in both Skilled Nursing Facility (SNF) and Outpatient settings in Sayler Park, Hamilton County, US. This full‑time role offers mentorship for new graduates, flexible scheduling and a supportive, person‑centered team.

Key Responsibilities

• Deliver comprehensive physical therapy evaluations, treatment plans, and progress notes for residents in the SNF.
• Provide outpatient services including therapeutic exercise, manual therapy, and functional training.
• Collaborate with physicians, nurses, and interdisciplinary team members to optimize patient outcomes.
• Educate patients, families, and staff on safe mobility techniques and discharge planning.
