Fidelis Care – New Jersey is seeking a registered nurse to join our team as a Care Manager (RN) in South Camden, Camden County. This remote/field‑based role involves extensive local travel to provide in‑person member support.

Key Responsibilities

• Conduct up to 75% travel for face‑to‑face member visits, assessments, and care coordination.
• Develop individualized care plans for members, ensuring alignment with clinical guidelines and Centene’s health transformation goals.
• Collaborate with interdisciplinary teams, including physicians, social workers, and community resources, to optimize health outcomes for a population of 28 million members.
• Monitor member progress, adjust interventions, and document outcomes in electronic health records.
• Educate members and families on disease management, preventive care, and available benefits.
• Maintain compliance with state and federal regulations, company policies, and quality standards.

Qualifications

• Active Registered Nurse (RN) license in New Jersey.
• Minimum 2 years of clinical nursing experience, preferably in case management or population health.
• Strong communication and interpersonal skills for building rapport with diverse members.
• Ability to travel extensively within the South Camden area and surrounding counties.
• Proficiency with electronic health record (EHR) systems and Microsoft Office suite.

Compensation & Benefits
